Introduction

Deep Plane Neck Lift

The neck can start showing signs of aging well before the face. If your neck looks fuller or your jawline lacks definition, a deep plane neck lift or deep structure neck lift can sculpt and refine the neck’s contours, delivering a dramatic improvement and giving you a more youthful appearance.

Dr. Gray’s isolated Deep Plane Neck Lift is a powerful and elegant solution for neck rejuvenation. Many patients struggle with a lack of neck definition despite being relatively young and active, have experienced weight changes, or have had nonsurgical treatments with underwhelming results. These patients often have neck fullness due to genetic looseness in muscle and glands rather than just excess fat, making them ideal candidates for a deep structure neck lift.

What is a Deep Plane Neck Lift?

A deep plane neck lift, or deep structural neck lift, is a neck and jawline sculpting procedure designed to address fullness and heaviness in the neck for younger patients (20s-30s) who have not yet experienced facial aging or midface descent.

Unlike traditional neck lifts which elevates and pulls the neck skin tightly behind the ears, the deep plane neck lift is a modern approach is performed through a single discrete incision under the chin. Dr. Gray targets the underlying platysma muscle and deeper neck structures, including deep fat, muscles, and glands to improve contour and sculpt a sharper jawline. The skin is then redraped over the newly contoured foundation, resulting in a natural, sharper neckline.

The deep plane neck lift is typically performed under general anesthesia, though it can be done under local anesthesia in select cases. In select cases, liposuction may also be used to remove excess fat above the platysma muscle.

What is the difference between a Deep Neck Lift and Neck Liposuction?

Neck liposuction targets fat between the skin and the platysma muscle, but on its own, it can sometimes yield disappointing results, especially if overdone, leading to a thin and streaky appearance.

Dr. Gray’s deep neck lift goes beyond the surface layer, working beneath the platysma muscle to sculpt deeper fat, muscle, connective tissue, and glands along the jawline for enhanced definition that liposuction alone cannot address.

After reshaping the deep neck structures, Dr. Gray performs a “platysmaplasty” to refine the angle between the chin and neck. The procedure is completed with a meticulously closed, small incision under the chin, which becomes nearly invisible as it heals.

In select patients, once the skin is closed, Dr. Gray will perform skin tightening technology which complements the deep neck lift, allowing the skin to adhere onto its new, elegant contour.

Dr. Gray’s deep neck lift technique precisely reshapes these underlying elements, creating a defined, youthful neck contour that simply isn’t possible with more superficial techniques.

DEEP NECK LIFT FAQs

Who is a candidate for a deep neck lift?

A isolated deep neck lift is ideal for younger patients in their 20s through 30s with genetic looseness and fullness in the neck area alone without excessive saggy skin, as the procedure sculpts the neck contours without addressing extra skin. For most patients past their 40s with extra, droopy skin, either an extended deep neck lift with or without deep plane facelift is more suitable.

Ideal candidates for an isolated deep neck lift should:

  • Be in good physical health
  • Have no serious medical conditions
  • Be non-smokers
  • Experience a loss of jawline definition
  • Have a genetic thicker or heavier neck contour
  • Maintain realistic expectations about the results of the procedure
What are the benefits of a deep neck lift?

An isolated deep neck lift offers distinct advantages for patients looking to sculpt and define their neck.

Key benefits include:

  • Eliminating a double chin
  • Enhancing jawline definition and shape
  • Improving the fit of open-necked or collared shirts
  • Contouring the neck for a more graceful, elegant appearance
  • Resulting in minimal scarring
  • Creating a more defined, sculpted jawline
  • Boosting self-confidence and self-esteem
How long do results last from a deep neck lift?

The results of an isolated deep neck lift are permanent and typically offers a dramatic improvement to the neck and jawline contour when fully healed. The skin, muscles, and fat continue to age however, as does the rest of the face.

Where are the incisions located for a deep neck lift?

A small incision located in the submental crease, or just under the chin, which is approximately 2-3 finger breadths wide, allows Dr. Gray access to the entire deep neck plane in order to sculpt the deeper structures. The under-chin incision is closed meticulously, healing near invisible.

No skin is removed with Dr. Gray’s isolated deep neck lift. Rather, then neck skin adheres to its new contour.

What is the recovery timeline for a deep neck lift?

Patients typically recover from a deep neck lift in 2 weeks, with average social downtime 5-10 days.

All patients heal from surgery on different timelines, however final results from a deep neck lift are expected at 3-4 months to allow swelling to come down.

Recovery timeline:

The Initial Days:

Following your procedure, you can go home with a friend or family member. Use cold packs around your face and neck for the first two days to minimize swelling. Keep your head and back elevated at a 45-60 degree angle while sleeping. If drains were used, they might be taken out after a few days. You can shower and wash your hair after two days, even if the drains are still in place.

The First Two Weeks:

During this period, focus on rest and recovery, but remember to walk around periodically to maintain good circulation and support healing. Keep sleeping in a slightly elevated position. Swelling may peak on the fourth day after your surgery but should start to decrease afterward. Avoid driving until you feel very comfortable and can safely perform driving tasks, which typically takes about a week. You may begin using makeup after the first week.

Week 2:

Bruising and swelling should show considerable improvement. Most patients feel ready to go out for errands or dining after about ten days. By the end of two weeks, you’ll likely be feeling better and can usually resume light work and activities, though some may return even sooner.

Week 3:

By the end of the third week, you can start engaging in moderate exercise. Swelling and bruising should show noticeable improvement.

Weeks 4-6:

Most patients can return to strenuous activities with approval from Dr. Gray. He will verify that your healing is complete before clearing you for vigorous exercise.

What happens before and during a deep neck lift?

Dr. Gray will conduct a thorough consultation with you to understand your aesthetic goals, assess your skin and neck, and explain the procedure in detail.

Your procedure will be performed under general anesthesia. Instead of traditional facelift incisions around the ears, for an isolated deep neck lift, Dr. Gray will make a single incision discreetly hidden beneath the chin.

Through this incision, he will lift the neck’s layers—including skin, muscle, fat, glands, and connective tissues—to sculpt a refined and elegant neckline and jawline. Dr. Gray will remove both superficial and deep fat in the neck and under the chin, tightening and lifting the neck muscles.

The isolated deep neck lift procedure will take approximately 2-3 hours of surgical time.

Deep Neck Tissue Contouring

Illustration Demonstrating Different Fat Layers in the Neck

The deep plane neck lift effectiveness lies in the deeper depth of dissection than traditional neck lifts, as Dr. Gray focuses on the plane under the platysma muscle, where the majority of patients hide deeper fat, sagging glands and tissue. The combination of tissue contouring along the jawline, platysma muscle tightening, and skin tightening treatment allows for a youthful, well-defined neck.

The diagram shows the how the fat above and below the platysma muscle and the sagging glands create an poorly defined neck.

Superficial (supraplatysmal) fat is addressed with Neck Liposuction which some patients are a candidate for.

Deeper (subplatysmal) fat is addressed with a Deep Neck Lift. Dr. Gray surgically sculpts the deep fat, muscle, and glands to sharpen and refine the angle between the chin and the neck.
