There has been a shift from traditional “SMAS facelifts” to extended deep plane facelifts due to their natural, longer-lasting results. The deep plane facelift is the most advanced lifting technique and now the gold standard for facelifting. Deep plane facelift moves beyond the superficial layers to reposition deeper facial structures, called the Superficial Musculoaponeurotic System (SMAS). Both deep plane and traditional facelifts target the SMAS layer, the connective tissue and fibrous network beneath the skin. However, their approaches to lifting and rejuvenating the face differ significantly.
Many patients desire a natural, refreshed appearance without the tight or pulled look commonly associated with older facelift techniques. This undesirable outcome often results from tension on the skin. Most U.S. plastic surgeons perform SMAS facelifts, which involve either folding back or trimming the SMAS layer and then tightening it with stitches. A deep plane facelift offers a different approach and excellent outcomes which is why facial plastic surgeons trained in the past decade prefer deep plane techniques. By carefully releasing the SMAS from underlying muscles and releasing all of the facial retaining ligaments, the deeper layers and also the skin can redrape to its prior, youthful position for a natural lift without skin tension.

While younger patients can benefit from a deep plane facelift in Boise, optimal candidates are typically age 40-70 men and women with pronounced facial and neck laxity. To achieve optimal results, patients should be in good health, nonsmokers, and possess realistic expectations.

While the face continues to age from the effects of gravity and volume loss after surgery, a deep plane facelift appears to shift back the timeline for a patient.
Deep plane facelift incisions are located in the same location as a traditional facelift, however since a deep plane facelift does not place tension on the skin, scaring is less than older facelift techniques.

Important to note that with facial rejuvenation procedures such as the deep plane facelift, Dr. Gray will often recommend undergoing complementary procedures to achieve a natural result and these may be included with the price or be priced separately. Procedures that can complement a facelift include upper and lower blepharoplasty, facial fat grafting, lip lift, chin augmentation, laser resurfacing, etc.
Patients typically recover faster from a deep plane facelift that other techniques. However, recovery from facelift surgery requires both social and professional downtime.
All patients heal from surgery on different timelines, but as a general guide, Dr. Gray recommends 2 to 3 weeks completely off from work (in person work/client meetings) or in person social events. 4-6 weeks planned for facial swelling/bruising to linger, and 12 weeks until you can be in the spotlight (i.e. on stage or wedding ready).
Recovery timeline:
The Initial Days:
Following your procedure, you can go home with a friend or family member. Use cold packs around your face and neck for the first two days to minimize swelling. Keep your head and back elevated at a 45-60 degree angle while sleeping. If drains were used, they might be taken out after a few days. You can shower and wash your hair after two days, even if the drains are still in place.
The First Ten Days:
During this period, focus on rest and recovery, but remember to walk around periodically to maintain good circulation and support healing. Keep sleeping in a slightly elevated position. Swelling may peak on the fourth day after your surgery but should start to decrease afterward. Avoid driving until you feel very comfortable and can safely perform driving tasks, which typically takes about a week. You may begin using makeup after the first week.
Week 2:
Bruising and swelling should show considerable improvement. Most patients feel ready to go out for errands or dining after about ten days. By the end of two weeks, you’ll likely be feeling better and can usually resume light work and activities, though some may return even sooner.
Week 3:
By the end of the third week, you can start engaging in moderate exercise. Swelling and bruising should show noticeable improvement.
Weeks 4-6:
Most patients can return to strenuous activities with approval from Dr. Gray. He will verify that your healing is complete before clearing you for vigorous exercise.
Your journey begins with a consultation where Dr. Gray assesses your facial concerns and goals. He will examine your skin laxity, facial volume loss, and skin quality to create a tailored treatment plan and any complementary surgical procedures to accompany the deep plane facelift.
You will receive a comprehensive explanation of the deep plane facelift procedure, including pre-operative care and required medications, and a presurgical clearance which involves cessation of smoking and blood-thinning medications and patients will get any needed medical workup prior to surgery.

Surgery involves general anesthesia before precise incisions are made. Dr. Gray will then release, lifts and reposition the SMAS layer, which is achieved after release underlying facial ligaments, and meticulously close the incisions.
