What is the difference between an Isolated Deep Plane Neck Lift and an Extended Deep Plane Neck Lift?

Isolated Deep Plane Neck Lift: Designed for younger patients in their 20s to mid 30s, who are at an appropriate body weight but have genetically heavy or poorly defined necks, who seek a dramatic neck and jawline transformation. This advanced and modern procedure utilizes a single incision under the chin to access and sculpt the deep neck structures, such as deeper fat, muscle laxity and glands. By addressing these foundational structures along the jawline, the Deep Plane Neck Lift provides substantial neck contouring and creates durable, natural-looking results. No skin is removed in this procedure.

Extended Deep Plane Neck Lift: Ideal for patients typically in their mid 30s to mid 40s experiencing mild to moderate neck laxity without significant jowling or midface descent. The procedure uses discreet incisions behind the ears and a small incision beneath the chin, allowing for precise deep neck contouring and removal of excess skin, resulting in a smoother, more defined neck contour.

While age is not the main factor, patients who are in their late 40s and 50s, the vast majority of individuals begin to have midface descent, or early jowling, which Dr. Gray will typically recommend a Deep Plane Facelift along with a Neck Lift. In these patients, a neck lift alone would appear unbalanced and accentuate jowling.

How Much Does A Neck Lift Cost in Boise, ID?

The price of both types of Deep Neck Lifts offered by Dr. Gray range from $6,500 to $8,500. The procedure chosen and subsequent price is determined by the neck rejuvenation techniques that Dr. Gray uses and any other treatments or procedures necessary to meet your aesthetic goals. Dr. Gray’s pricing includes surgeon fees; however, facility and anesthesia fees can be variable and based on the anticipated procedural time.

The best way to find out how much it will cost for you is to come in for a personalized visit and let Dr. Gray create your surgical plan once an in-depth consultation process is complete. Once a treatment plan is agreed upon, our team will be able to provide a detailed quote detailing the price breakdown for your procedure and offer financing options where appropriate.

Modern Neck Rejuvenation Techniques

What conditions does a neck lift address?

With a neck lift in Boise, patients can effectively address various signs of aging in the lower face and neck, including:

Loose Skin: Over time, skin loses elasticity, leading to sagging and wrinkles in the neck area. Neck lift surgery removes excess skin, resulting in a tighter, smoother appearance.

Platysma Bands: The platysma muscle in the neck can become visible as vertical bands with age. Neck lift surgery tightens and smooths this muscle, significantly reducing the appearance of these bands.

Loss of Definition: Aging can cause the jawline to lose definition, creating a “turkey wattle” appearance under the chin. A neck lift redefines the jawline, eliminating this waddle and restoring a more youthful, contoured neck.

Gray Facial Plastic Surgery

Why choose Dr. Gray for your surgical neck lift?

Dr. Gray specializes in advanced neck rejuvenation techniques that go beyond traditional approaches to deliver natural, long-lasting results. With a foundation in head and neck surgery and fellowship training at one of the nation’s highest-volume neck lift centers, Dr. Gray brings deep expertise to each procedure. During his fellowship year alone, he participated in over 200 neck lift surgeries.

Modern neck rejuvenation is no longer limited to liposuction or surface-level fat removal. Today’s most effective results come from addressing the deeper structural causes of neck aging—such as enlarged and sagging submandibular glands, thickened and bulky neck muscles, and excess deep fat. Dr. Gray’s deep structure neck lift technique precisely reshapes these underlying elements, creating a defined, youthful neck contour that is simply not possible with older techniques.

NECK LIFT FAQs

Who is a candidate for a neck lift?

An Isolated Deep Neck Lift alone is ideal for younger patients in their 20s to 30s with genetic fullness in the neck area alone without excessive saggy skin, as the procedure lifts the neck contours without addressing loose, sagging skin.

For patients past their mid 30s to 40s with extra, droopy skin, an Extended Deep Neck Lift may be appropriate which offers excess skin removal.

Beyond this, once patients have signs of midface descent or jowling, a Deep Plane Facelift combined with a neck lift is more suitable.

Ideal candidates for a neck lift should:

  • Be in good physical health and near ideal body weight
  • Have no serious medical conditions
  • Be non-smokers
  • Experience a loss of jawline definition
  • Have a thicker or heavier neck contour
  • Maintain realistic expectations about the results of the procedure
What are the benefits of a neck lift?

A neck lift offers distinct advantages for patients looking to sculpt and define their neck.

Key benefits include:

  • Eliminating a double chin
  • Enhancing jawline definition and shape
  • Improving the fit of open-necked or collared shirts
  • Contouring the neck for a more graceful, elegant appearance
  • Resulting in minimal scarring
  • Creating a more defined, sculpted jawline
  • Boosting self-confidence and self-esteem
How long do results last from a neck lift?

The results of a neck lift are permanent and typically offers a dramatic improvement to the neck and jawline contour when fully healed. The skin, muscles, and fat continue to age however, as does the rest of the face.

Where are the incisions located for a neck lift?

Isolated Deep Neck Lift: A small incision located in the submental crease, or just under the chin, which is approximately 2-3 finger breadths wide, allows Dr. Gray access to the entire neck and the deeper structures as he works through this “access incision”. The incision is closed meticulously, healing near invisible.

Extended Deep Neck Lift: The same incision under the chin is used, and additionally there are small incisions below and behind the ear which hide well. This allows Dr. Gray access to the side of the neck to remove excess skin and complete a lateral neck lift, address the center and sides of the neck for complete rejuvenation.

What is the recovery timeline for a neck lift?

Patients typically recover from a neck lift in 2 weeks, with average social downtime 7-10 days.

All patients heal from surgery on different timelines, however final results from a neck lift are expected at 3-4 months to allow swelling to come down.

Recovery timeline:

The Initial Days:

Following your procedure, you can go home with a friend or family member. Use cold packs around your face and neck for the first two days to minimize swelling. Keep your head and back elevated at a 45-60 degree angle while sleeping. If drains were used, they might be taken out after a few days. You can shower and wash your hair after two days, even if the drains are still in place.

The First Two Weeks:

During this period, focus on rest and recovery, but remember to walk around periodically to maintain good circulation and support healing. Keep sleeping in a slightly elevated position. Swelling may peak on the fourth day after your surgery but should start to decrease afterward. Avoid driving until you feel very comfortable and can safely perform driving tasks, which typically takes about a week. You may begin using makeup after the first week.

Week 2:

Bruising and swelling should show considerable improvement. Most patients feel ready to go out for errands or dining after about ten days. By the end of two weeks, you’ll likely be feeling better and can usually resume light work and activities, though some may return even sooner.

Week 3:

By the end of the third week, you can start engaging in moderate exercise. Swelling and bruising should show noticeable improvement.

Weeks 4-6:

Most patients can return to strenuous activities with approval from Dr. Gray. He will verify that your healing is complete before clearing you for vigorous exercise.

What happens before and during a neck lift?

Dr. Gray will conduct a thorough consultation with you to understand your aesthetic goals, assess your skin and neck, determine which neck lift is suited for you, and explain the procedure in detail.

Your procedure will be performed under general anesthesia. Dr. Gray will make a single incision discreetly hidden beneath the chin in a deep plane neck lift, and for an Extended Deep Neck Lift, discrete incisions below and behind each ear.

Through the incision(s), he will lift the neck’s layers—including skin, muscle, fat, glands, and connective tissues—to sculpt a refined and elegant neckline and jawline. Dr. Gray will remove both superficial and deep fat in the neck and under the chin, tightening and lifting the neck muscles.

The neck lift procedure will take approximately 2-3 hours of surgical time.

Deep Structural Tissue Contouring

Illustration Demonstrating Different Fat Layers in the Neck

Dr. Gray’s neck lift effectiveness lies in the deeper depth of dissection than traditional neck lifts, as Dr. Gray focuses on the plane under the platysma muscle, where the majority of patients hide deeper fat, sagging glands and tissue. The combination of tissue contouring along the jawline, platysma muscle tightening, and skin tightening treatment allows for a youthful, well-defined neck.

The diagram shows the how the fat above and below the platysma muscle and the sagging glands create an poorly defined neck.

Superficial (supraplatysmal) fat is addressed with Neck Liposuction which some patients are a candidate for.

Deeper (subplatysmal) fat is addressed with a Deep Neck Lift. Dr. Gray surgically sculpts the deep fat, muscle, and glands to sharpen and refine the angle between the chin and the neck.
